New Patient Forms

Most doctor’s offices have you fill in extensive forms before your visit. Dr. Yarnell prefers to go through these with you in person, so there will only be a small number of brief forms to fill out when you arrive for your appointment.

Other Information We Request

Please bring your the actual bottles of any supplements or herbs you are currently taking so Dr. Yarnell can see the exact ingredients. Please bring a list of all medications you are currently taking. If you have any medical records, getting those to us ahead of your visit will save time and make the visit go more smoothly. Also please bring a list of any other health care providers you are currently working with, along with their contact information.


All visits must be paid for at the time of service. We will bill your insurance or can accept payment at the time of service. Please contact the clinic of your choice regarding costs.

Office Hours

Dr. Yarnell is in Practitioner Care at the Bastyr Center for Natural Health clinic Tuesdays from 8:30 am to 5 pm. He is in Team Care supervising students at the Bastyr Center for Natural Health clinic Thursdays from 8:45 am to 12 pm. He is in the Bastyr Integrative Oncology Research Center from 12 to 4 pm on Mondays.
